For Type I and II achalasia, POEM and Heller plus fundoplication are both effective options in suitable patients and should be discussed through shared decision-making.
Why add fundoplication?
Myotomy can weaken the anti-reflux barrier, so partial fundoplication is commonly added during surgery to reduce reflux risk.

Reflü avantajı ne demek?
Heller miyotomisine eklenen fundoplikasyon reflüyü azaltmayı amaçlar; ancak reflü riski tamamen sıfırlanmaz. İşlem sonrası semptomlar ve gerektiğinde objektif testlerle takip yine önemlidir.
